(A) The Board of Health of Cass County shall charge a fee of $10 for the issuance of a birth certificate; $10 for the issuance of a death certificate and for the issuance of a wallet size, laminated birth certificate.
(B) The Board of Health of Cass County shall charge a fee of $5 for each record search.
(C) The Board of Health of Cass County shall charge a fee of $20 for each of the following:
(1) Affidavit of legitimation;
(2) Paternity affidavit; and/or
(3) Affidavit requesting amendment.
(D) The Board of Health of Cass County shall charge a fee of $100 for the issuance of a septic permit.
(E) The Board of Health of Cass County shall charge a fee of $50 for issuance of an installer certification. This fee must be submitted prior to the issuance or renewal of the certification.
(F) The Board of Health of Cass County shall charge a fee of $25 for the issuance of an annual pool permit.
(G) (1) The Board of Health of Cass County shall charge a fee of $100 for the issuance of an annual food permit for "for profit" establishments.
(2) Establishments that sell only prepackaged food and do not engage in food handling shall be charged $50 for the issuance of an annual food permit.
(H) The Board of Health of Cass County shall charge a fee of $20 per day, with a maximum charge of $100, for the issuance of a temporary food permit.
(I) The Board of Health of Cass County shall charge a fee of $50 for a late charge if the annual food permit is not paid by January 31.
(J) The Board of Health of Cass County shall charge a fee of $5 for a TB (Tuberculosis) test. There shall be no charge for tests related to contact investigations.
(K) The Board of Health of Cass County shall charge a fee of $15 for a flu shot (Influenza).
(L) The Board of Health of Cass County shall charge a fee for "travel immunizations" as follows:
(1) Yellow Fever (boost q 10 years): $90;
(5) Typhoid (boost q 2 years): $60;
(6) Tetanus/diphtheria (TD) (over 18 yrs): $25; and/or
(7) Inactivate polio vaccine (IPV) (over 18 yrs): $30.
(M) The Board of Health of Cass County shall charge a fee of $250 annually beginning January 1, of each year for a tattoo business permit.
(N) The Board of Health of Cass County shall charge a fee of $50 annually for a tattoo artist permit.
(O) The Board of Health of Cass County shall charge a fee for the following vaccinations, as indicated:
(1) Chicken Pox (Varicella) (over 18 yrs): $100 (children are free);
(2) MMR (over 18 yrs): $60 (children are free);
(3) Pneumonia (over 18 yrs): $60;
(4) Tdap (Adacel) (over 18 yrs): $45;
(5) HPV Vaccine (Gardasil) (over 18 yrs): $150; and/or
(6) Shingles Vaccine (Zostavax): $170.
(P) The Board of Health of Cass County shall charge a fee of $.10 per page for copying documents kept by the Department.
